Important Notice:

All applications will be subject to a comprehensive screening process, which may take up to four (4) weeks upon receipt of the complete documentation and payment. Updates regarding the status of your application will be communicated via email.

Please ensure that the applicant’s details are consistent with the details in the Annual Practicing Certificate (APC).

The required documents for this screening process are:

  • A copy of applicant’s NRIC (front and back)
  • A copy of latest applicant’s APC
  • Evidence of registration with the Ministry of Health Malaysia under the Private Healthcare Facilities and Service Act 1998 and Private Medical Clinics (Borang B/F)

Sign Up as an X-ray Facility Provider

Are you interested in becoming one of FOMEMA’s panel X-ray facility in carrying out chest X-ray examination of foreign workers?
You may sign-up here

Important Notice:

All applications will be subject to a comprehensive screening process, which may take up to four (4) weeks upon receipt of the complete documentation and payment. Updates regarding the status of your application will be communicated via email.

Please ensure that the applicant’s details are consistent with the details in the Annual Practicing Certificate (APC) and X-ray licence.

The required documents for this screening process are:

  • A copy of applicant’s NRIC (front and back)
  • A copy of applicant’s latest APC
  • Evidence of registration with the Ministry of Health Malaysia under the Private Healthcare Facilities and Service Act 1998 and Private Medical Clinics (Borang B/F)
  • A copy of the applicant’s X-ray licence along with the Lampiran A of the document

Important Notice:

All applications will be subject to a comprehensive screening process, which may take up to four (4) weeks upon receipt of the complete documentation and payment. Updates regarding the status of your application will be communicated via email.

The required documents for this screening process are:

  • Copy of Certificate of Incorporation (SSM Certificate)
  • Copy of EQA Programme Subscription document
  • Copy of MS ISO 15189:2022 Accreditation document
  • Copy of document on Skim Akreditasi Makmal Malaysia (SAMM) for all FOMEMA tests
  • Copy of utility bill of the laboratory premises
  • Copy of impression of the laboratory’s official rubber stamp
