This digital process took effect on 2 March 2026. From this date onwards, employers should no longer use manual forms and must submit requests directly through the Employer section of the FOMEMA Online Portal.
Submissions are now fully digitised.
Employers may submit a request directly through the Employer section of the FOMEMA Online Portal during the clinic selection process.
Please log in to the portal, proceed with clinic selection, and click the “Increase Quota” button to submit the request.
The “Increase Quota” button is displayed during the clinic selection process. Its status depends on quota availability in the selected area:
If all clinics in the selected area have reached their quota, please follow the steps below:
Step 1: Select the State and Town, enter the Postcode, and click the Search button
Step 2: Click the Increase Quota button
Step 3: Tick worker(s) and click Submit button

Once the “Increase Quota” button is enabled:
All requests are submitted to the Ministry of Health Malaysia (MoH) for consideration and decision.
FOMEMA facilitates the submission and processing of the request through the FOMEMA Online Portal.
An automated email notification will be sent to employer’s registered email address within seven (7) working days from the date of submission through the FOMEMA Online Portal.
Upon receiving an automated email notification via the FOMEMA Online Portal, employers will be informed of their request result.
Employers may track the request status by navigating to the “Others” tab and select “Request Doctor Quota”.
The status will be displayed as one of the following:
Pending Approval: The request has been submitted and is currently being reviewed by the Ministry of Health Malaysia.
Approved: The request has been approved, and employers may proceed with the clinic selection.
Rejected: The request was not successful. Employers are advised to select a clinic in the nearest available area.
If the request is rejected, employers should proceed with clinic selection using the nearest available clinic or broaden the search by selecting another state, town, or postcode.
The “Increase Quota” button remains disabled if there are still clinics with available quota in the selected area.
Effective 1 January 2022, the quota is distributed quarterly as follows:
Total quota per doctor per year: 500 foreign workers.
The employer should first register two workers using the available quota.
Once the quota in the selected area has been fully utilised, the “Increase Quota” button will be enabled, allowing the employer to submit a request for the remaining workers.
