The foreign worker FOMEMA Medical Examination Form expires between 22/03/2025 to 04/04/2025, and the clinic is located in one of the listed affected areas.
No application is required. Eligibility will be automatically granted based on the customer’s area or clinic’s location within the listed region.
No documents are required. The form validity will be automatically extended if you meet the specified criteria.
No notification will be sent as eligibility is automatic if the foreign worker FOMEMA Medical Examination Form expires within the specified date range and the clinic is in one of the listed affected areas.
No penalties will be imposed, but the form will expire and need to be repurchased if not used within the extended period.
