To sign up as an agency, please provide the following documents:

1. A copy of your company’s registration document (SSM Form 9)
2. A copy of your company’s registration document (SSM Form 49)
3. A copy of the JTKSM License
4. A copy of your Director’s NRIC

The agency fees are:

• RM300 registration fee
• RM100 renewal fee (every two years)

After your registration is approved, you can login to the FOMEMA Online Portal and make the fee payment before gaining access to your account.
