The NCD Monitoring Programme is a FOMEMA initiative developed in alignment with Ministry of Health (MoH) guidelines to support the ongoing health management of foreign workers in Malaysia diagnosed with approved Non-Communicable Diseases (NCDs).
Designed for workers identified through appeal cases, the programme provides continuous medical monitoring, follow-up care, and access to appropriate treatment. Its objective is to enable these workers to remain employed in Malaysia while effectively managing their health conditions under a structured list of diseases approved by FOMEMA.
The NCD Monitoring Programme was introduced to give employers an opportunity to retain their foreign workers who have been diagnosed with certain NCDs that are manageable with medication and lifestyle changes. It supports fair employment practices by allowing eligible workers to remain in Malaysia under structured health monitoring.
Introduced in accordance with the guidelines set by the MoH, the programme aims to enhance long-term health management among the foreign workforce. It also helps employers maintain workforce stability and productivity, offering a more cost-effective alternative to hiring and training new workers.
The programme is a long-term initiative established under the directive from the MoH. It aims to ensure that foreign workers with NCDs receive ongoing health management including medical supervision and necessary care.
This programme is intended to remain in place for as long as there is a need to support the ongoing health management of foreign workers diagnosed with non-communicable diseases.
The programme ensures that affected workers continue to receive proper medical supervision and necessary care throughout their employment in Malaysia.

This programme is only mandatory for applicable cases, specifically foreign workers with NCDs identified through the appeal process as requiring further medical monitoring.
The following Non-Communicable Diseases (NCDs) are included in the mandatory Monitoring Programme:
•Diabetes
•Hypertension
•Kidney Disease
•Heart Disease
•Other related conditions

Foreign workers who have completed the appeal process will be reviewed to determine whether participation in the NCD Monitoring Programme is required.
Foreign workers who are diagnosed with the Non-Communicable Diseases (NCDs) approved under the programme may be enrolled in the NCD Monitoring Programme.
Enrolment applies to those identified by a medical specialist as requiring ongoing treatment, follow-up, or medication during the appeal process.
Once a foreign worker is identified eligible to participate in the programme, the employer or agency will receive an official email notification from FOMEMA to proceed with payment and initiate the enrolment process.
The programme ensures that affected foreign workers continue to receive proper medical supervision and necessary care throughout their employment in Malaysia.
It supports employer by helping to maintain workforce stability and productivity, offering a more cost-effective alternative to hiring and training new workers.
The programme covers a range of NCDs, including (but not limited to):

There is no enrolment fee for the NCD Monitoring Programme. However, there will be other charges may vary based on the NCD of the foreign worker.

Payments can be made using a variety of methods, including Debit and Credit Cards, Online Banking (FPX B2B and B2C), QR Payment and e-Wallets.
Yes. Payment must be made within seven (7) days from the date of the email notification.
Employer or Agency is encouraged to make payments via the FOMEMA Online Portal. You are required to log in to complete the transaction.
If payment is not made within the stipulated period, the appeal process will be closed, and the appeal case will be considered as unsuccessful.
In this event, the Employer or Agency must go through the registration process again to make the payment.
To do this, the Employer or Agency is required to send an email to appeal@fomema.com.my to request for manual re-registration of the appeal.
Once the manual registration is completed, a FOMEMA officer will respond to the email within five (5) working days.
The review and approval process takes three (3) to five (5) working days from the date of payment.
Yes. A receipt can be generated and downloaded from the FOMEMA Online Portal upon successful payment.
Once payment is confirmed, the foreign worker will be enrolled in a six-month monitoring programme.
This programme will be facilitated exclusively through the WellXS mobile application, a wellness platform that provides foreign workers with access to essential health services. Employers and Agencies must download and register on WellXS, as it serves as the primary platform for managing and tracking foreign workers’ health status throughout the programme.
In the fifth month of the monitoring period, a notification will be sent via the app, prompting the Employer or Agency to select a hospital or clinic for the foreign worker to undergo a medical check-up. This will be based on the NCD category indicated in the earlier payment.
The payment amount varies depending on the type and severity of the NCD. Different conditions may require different levels of monitoring, treatment and follow-up, which affects the overall cost.

Employer or Agency is required to make direct payments to the clinic or hospital for any medication or treatment provided under this programme.
Please note that the NCD Monitoring Programme fees paid to FOMEMA cover only the follow-up tests, not treatment or medication.
No additional charges will be imposed for the regular monitoring process as all standard costs are collected upfront by FOMEMA.
However, in cases involving heart disease or conditions that require specialist assessment, additional fees may apply. These charges will be based on the specialist’s recommendations and are subject to prior agreement by the Employer or Agency.
The NCD Monitoring Programme enrolment fees are non-refundable, regardless of changes in the worker’s employment status.
There will be no restrictions for foreign workers under this programme to travel outside Malaysia.
Yes, they can. Once the foreign worker’s status is updated to ‘Suitable’, they can proceed their PLKS application, even during the programme monitoring period.
There will be no changes made to the foreign worker’s medical examination status throughout the six-month monitoring period.
Foreign workers identified with the eligible NCDs is encouraged to enrol in the NCD Monitoring Programme.
If no payment is made within the stipulated time frame, then the status of the foreign worker will remain as ‘Unsuitable’.
